CANCELLATIONS

All cancellations must be made no less than 48 hours before the scheduled appointment.*
Late Cancellations/No-Shows: A cancellation is considered late when made 48 hours or less before a
scheduled appointment. A no-show occurs when an appointment is missed.*

CANCELLING YOUR SCHEDULED APPOINTMENT

To cancel your existing appointment, please complete the TrustAssure Cancellation Request form on-
line.

To cancel your appointment, you will need your Appointment Confirmation Number, which can be found
in your TrustAssure appointment confirmation email.

RESCHEDULING A BOOKED APPOINTMENT


TrustAssure does not currently support the ability to reschedule an appointment on your behalf.

For same-day reschedule requests at the original booking location, you will need to contact your
test site location directly to request a change.

If you need to reschedule an existing appointment and are within the TrustAssure cancellation
window, please cancel your existing appointment and book a new appointment.


NOTE: All other cancellations may be subject to a no-show fee.


PLEASE NOTE: TrustAssure does not collect personal or financial information from passengers over the
phone and does not book, reschedule or cancel lab appointments for customers over the phone.
