TrustAssure does not accept insurance as payment for any testing appointment. Should you wish to
submit your claim to your insurance directly, please use your original receipt and confirmation.
TrustAssure is unable to provide additional information or breakdown needed for claims. The additional
information needed for claims may be found on your report.
If you have not received a lab result/report, please contact your lab provider.

Lab Result:
• Name and Address of Billing Provider
• Patient Name
• Type of Service Provided
• Result of Diagnosis
• Date of Service
• Billing provider NPI and Tax ID
Receipt:
